New Patients
We welcome new patients to our surgery and as it is important for our doctors to understand all of your health needs past and present, you may be asked to book a longer appointment for their first appointment. You can assist us by completing a New Patient form and New Patient History Form and bringing these along to your first visit. These forms are available to print on the forms page.
Sometimes your new doctor may wish to access a copy of your medical record from your previous practice. The transfer of medical files is commonly done and we can assist you to arrange this. Please be aware that some practices may charge a small administrative fee for a file transfer to cover the associated administrative and postage costs. Your previous practice will advise you if there will be a transfer fee and how much.
Fees
Singleton Heights Medical Practice is a combination Private and Medicare Bulk Billing practice.
We use AMA guidelines to set our fee structure and recognise these government concession cards:
- Current Pensioner Concession Cards
- Current Health Care Cards
- Current DVA Cards
- Current Commonwealth Senior Cards
Most of our doctors on site will bulkbill concession card holders and children under 16 years of age. Whilst other doctors will offer a reduced rate for concession card holders.
Please check when making your appointment to see if you qualify for bulkbilling.
Please make sure that your bank account details are registered with Medicare so that your benefits will be directly paid into your account.
Private Fees
- Level A $50.00 Brief Consultation
- Level B $90.00 Normal Consultation
- Level C $160.00 Long Consultation
Enquiries regarding costs for pathology, scans, x-rays and other tests should be made when booking the procedure/appointment
There may also be associated costs with services provided within the treatment room:
- Dressings
- Fibreglass casts, Waterproof cast lining
- Immunisations (other than those provided on the immunisation free list)
- Lesion Removal
- ECG’s
- Spirometry
- Medicals
Emailing The Practice
Our emails are checked daily by reception and distributed to the required person. Please DO NOT request medical appointments via emails as we may not be able to triage your medical needs appropriately. We are very happy for you to email us with other enquiries.
We also cannot send confidential patient information to patients private or business email addresses due to non-secure messaging service. (Medical encryption)
Privacy of your Health Information
All staff, including administrative staff respect your privacy and keep your health information confidential at all times. Your medical record is a confidential document and it is policy of this practice to maintain the security of your personal health at all times.
There is a Health Information Consent Form listed in the contact us section for you to complete and return to reception for inclusion in your health records.
If you have any concerns about the use of your personal health information, please speak with your GP or our Practice Manager.
A copy of our practice Privacy Policy is available at reception.
From time to time your personal health information may be collected, used and disclosed for the following reasons:
Communicating with other treating doctors and clinical staff within our practice and on referral to specialist or Allied Health Professionals.
For notifying you about health services that may be due or require follow up with your GP.
For inclusion on national and State registers i.e. Immunisation, Pap reminders.
For Medicare purposes.
Disease notification as required by law.
For legal disclosure, as by a court of law.
After Hours Medical Help
Singleton Heights Medical Practice shares an after hour’s cover through an “On Call” arrangement at Singleton District Hospital.
Patients can either contact Singleton District Hospital Emergency Department phone: 02 6571 9222 for medical advice or present in person to the hospital located on Dangar Road Singleton.
Home Visits
When required our Doctors visit aged care facilities and private homes and these visits normally take place during surgery hours.
Home visits must be located within a reasonable distance within the Singleton Township.
Home visits are at the discretion of the Doctor. These visits will be triaged through the Registered Nurse and/or Doctor.
